Zygomatic Implants
An Advanced Solution for Severe Upper Jaw Bone Loss
A Proven Alternative to Extensive Bone Grafting
Patients with significant bone loss in the upper jaw are often told they are not candidates for dental implants. Zygomatic implants offer a predictable, graftless solution, anchoring long implants in the cheekbone (zygomatic bone) rather than in the maxilla.
At Star Oral Surgery, Dr. J. Scott Bobst performs this advanced procedure for patients who require full-arch rehabilitation but lack sufficient bone for traditional implants. With proper planning and execution, zygomatic implants can support immediate prosthetic placement and deliver life-changing functional and aesthetic outcomes.
What Are Zygomatic Implants?
Unlike conventional dental implants, which are placed vertically into the jawbone, zygomatic implants are longer, angled implants that engage the dense zygomatic bone (cheekbone) for stability. This allows patients with severe maxillary resorption to receive full-arch implant-supported prosthetics, often without the need for bone grafting or sinus lifts.
Zygomatic implants are typically part of a hybrid treatment plan that may also include traditional or pterygoid implants, depending on bone anatomy and prosthetic requirements.
Candidates for Zygomatic Implants
Zygomatic implants may be indicated for patients who:
- Have severe bone loss in the upper jaw
- Have been told they are not candidates for standard implants
- Wish to avoid sinus lift or extensive bone grafting
- Are seeking full-arch rehabilitation (All-on-4® or similar solutions)
- Wear removable dentures and are looking for a fixed, long-term solution
A full clinical evaluation and 3D CBCT imaging are required to determine candidacy.
The Procedure
Zygomatic implant surgery is performed in a fully equipped surgical suite under IV sedation or general anesthesia, depending on the case complexity. Using 3D-guided planning, Dr. Bobst places the implants at a precise angle to engage the zygomatic bone while preserving adjacent anatomical structures.
In many cases, a fixed temporary prosthesis is placed immediately after surgery, allowing patients to leave with a full-arch restoration the same day. Final prosthetics are delivered after full integration, typically within 3 to 6 months.
Post-Operative Recovery
Recovery following zygomatic implant placement is similar to that following other full-arch procedures, but due to the implant trajectory, it may involve slightly more swelling or sinus sensitivity.
Patients should expect:
- Swelling peaking within 48–72 hours
- A modified soft-food diet for several weeks
- Activity restrictions and medication adherence as instructed
- Ongoing hygiene protocols and follow-up evaluations
