Bone grafting is an essential first step for many patients preparing for dental implants. Whether your graft supports a future implant site or is part of a more complex reconstruction, proper post-operative care is critical to ensure success.
At Star Oral Surgery, we use high-quality graft materials and proven techniques to help your jawbone regenerate. Now, it’s time to help that process along by following these care instructions closely.
What to Expect After Bone Grafting
Swelling, mild bleeding, and discomfort are normal during the first few days after surgery. Most symptoms peak by day 2–3 and then begin to subside.
- The surgical site may feel raised or firm—this is the graft material settling into place.
- Small granules may come loose, especially during the first few days. This is not unusual, but call us if it seems excessive.
- Swelling or bruising around the cheeks or under the eyes may be more pronounced than a typical extraction.
Immediate Care (First 24 Hours)
- Do not disturb the surgical site; avoid touching, brushing, or rinsing directly over it.
- To protect the clot and prevent graft disturbance, avoid spitting, rinsing, or using a straw for 24 hours.
- Bite gently on gauze to manage bleeding, changing it every 30–45 minutes as needed.
- Apply ice packs on the outside of your face in 20-minute intervals to reduce swelling.
- Get plenty of rest and keep your head elevated to promote clot formation and minimize inflammation.
Medications
- Take pain relievers as prescribed or as directed by our office. Over-the-counter ibuprofen or acetaminophen can be effective.
- If antibiotics are prescribed, take the full course to prevent infection.
- Avoid aspirin unless directed, as it may increase bleeding.
- If you’re given a decongestant or nasal spray (for sinus lift grafts), follow the specific instructions we provide.
Diet & Oral Care
- For the first few days, stick to soft, cool foods. Avoid anything that could disturb the graft or lodge in the surgical area.
- Recommended foods: smoothies (no straw), yogurt, mashed potatoes, soup (lukewarm), scrambled eggs, soft pasta
- Avoid: hot foods, hard/crunchy textures, seeds, nuts, spicy items, and alcohol
- Do not brush or rinse the surgical site for the first 24 hours.
- Starting day 2, gently rinse with warm salt water (¼ tsp salt in 8 oz water) 2–3 times daily, especially after eating.
- Continue to brush and floss other areas of your mouth as usual, but carefully avoid the graft site.
Additional Considerations (for Sinus Grafts)
If your bone graft involved a sinus lift or work near the upper jaw:
- Do not blow your nose for at least one week.
- Avoid sneezing through your nose—sneeze with your mouth open to reduce pressure.
- Refrain from using straws, smoking, or heavy lifting.
- Use any nasal sprays or decongestants as directed by our office.
Healing Timeline
- Day 1–3: Swelling and discomfort peak, gradually subsiding
- Week 1: Soft tissue begins to close and stabilize
- Weeks 2–4: Graft site continues to heal internally
- Months 3–6: Bone integrates with your natural jaw tissue, preparing for future implant placement

When to Call Us
Contact us if you experience:
- Persistent bleeding beyond 24 hours
- Fever or signs of infection
- Severe swelling that worsens after day 3
- Unusual drainage or bad taste in the mouth
- Sinus complications (pressure, nosebleeds, or congestion that doesn’t improve)
